The other side
The man with violent thoughts stops mentioning them. That is what disclosure buys. Once a physician becomes a possible reporter, the sickest patients edit themselves, and the warning signs never reach a clinician at all. Confidentiality is the only reason people admit to the things worth intervening on. Break it predictably and the disclosures dry up before anyone can act.
